Ohio Residents, If you are operating in the state of OHIO, please review operation requirements at http://www.cos.ohio.gov/Home/Tanning
Attn: Ohio Tanning Salon Owners and Tanners: Parental consent needed for all under 18 years of age effective June, 2003. - Take the Test
Please Note: After you have completed the Ohio Online Program and successfully passed the online test with a score of 75% or better, you will then be required to complete a final test at one of our predetermined test sites across Ohio. - Contact one of our state testing sites to arrange your required monitored final exam.
You will be required to show a government-issued photo ID and a copy of your online receipt.
